Design Assumptions and Limitations Index 125-001 provides the minimum requirements for the excavation and backfill of utilities placed below existing pavement on the State Highway System, including requirements for replacement of the pavement. Index 125-001 is intended to be used by Maintenance and utility permit operations. Department owned utilities (i.e., conduit and drainage pipe) placed during construction are to be installed and paid for in accordance with their respective Standard Plans and Specifications .
